Applicants
Application Form "Accredit UK Ltd" is a business registered in England No 4850531. "Client" is the party requesting goods or services from Accredit UK Ltd. A contract is made upon acceptance by Accredit UK Ltd of appropriate authorisation from the Client such as a purchase order, official email or a completed application form.
The work as agreed between Accredit UK Ltd and the Client covered by this application will cover some or all of the following; certification documentation supplied to the Client, the visit, the outcomes, the assessor's report and action plan, consultancy, light touch review and Accredit UK Ltd's ongoing support in each year of the certification.
Where a visit date is not supplied on this agreement the Accredit UK Ltd Assessor will assign a visit date not less than 15 working days after the application date. Once a visit date has been agreed either by the Client or assigned by the Accredit UK Assessor this will be confirmed by email and clause 4 becomes effective.
By Client
The Client is allowed a maximum of one (1) reschedule request on the following terms based on the number of working days from the confirmed site visit:-
For reschedules made with greater than 4 weeks notice prior to the date of the visit, a fee of £750 will be payable.
For reschedules within 4 weeks prior to the date of the visit, or in the case of non attendance, a fee of £1500 will be payable.
Further reschedule requests will be charged at 100% of the fee.
Reschedule requests must be confirmed in writing by the Client and fees will be calculated from the received date of the written confirmation.
By Accredit UK Ltd
In the event that Accredit UK Ltd has to cancel a site visit, the Client will be offered an alternative booking.
This agreement shall continue until terminated by either party.
Certification
The initial assessment visit must be booked within 2 months of date of application (up to 6 months can be arranged with mutual consent). Certification is awarded for a 3 year period with annual "Light Touch" Reviews. Both Renewal and Review visits will be scheduled as close as possible to the anniversary of Certification.
Action Plan
An action plan will be provided after the initial Certification visit and renewed at each "Light Touch" review to ensure continuous improvement.
Light Touch Review
The Client maintains Certified status provided any actions identified in the action plan are completed by the "Light Touch" Review (or justification discussed with and agreed by the assessor).'
Working Towards
Businesses that are committed to the Accredit UK Ltd principles, but cannot yet demonstrate the required level of evidence based compliance, may apply for the Working Towards mark. Such businesses demonstrate that they are preparing for certification over a 12 month period. This level of recognition is awarded for a maximum of 12 months within which the Applicant must either book a full certification visit or withdraw from the Standard.
By signing this Application Form, the applicant agrees to pay the relevant fees - please contact us for pricing information. Payment is due on order, or, in the case of an approved credit account transaction supported by the Client's Purchase Order, payment is due within 14 days of invoice. Payment for reschedule charges is due within 14 days of invoice. In any event, all invoices must be paid in full prior to receiving the Application pack or certification visit to the Client.
Payment may be made by credit card, cheque or BACS.
Accredit UK Ltd reserves the right to charge interest on overdue monies at a compounded rate of 2% per calendar month on overdue invoices.
Any variations to these terms and conditions must be in writing and signed by both parties.
The applicant is not responsible for expenses incurred by Accredit UK Ltd staff or assessors relating to the site visit. This includes mileage and/or necessary public transport, accommodation and subsistence expenses.
The applicant agrees to adhere to Accredit UK's terms and conditions.
The contract shall be subject to English Law and the contracting parties submit to the exclusive jurisdiction of the courts of England.
All information/documentation received (except that already in the public domain or by prior written agreement with applicant) will be kept in confidence by Accredit UK. By sending this application form, the applicant accepts all the terms and conditions as above.
